Effects of physiological dead space10/10/2023 ![]() ![]() While some studies showed no association, 5, 12, 13 others found an association of the level of hypoxemia with mortality. 5, 8– 11 However, such association was controversial when evaluating the level of hypoxemia and mortality in early ARDS. The increase in the dead-space fraction is a risk factor associated with increased mortality during the early and intermediate phases of ARDS. Clinical manifestations include ongoing hypoxemia, bilateral opacities on the chest radiograph, fever, and leukocytosis, without evidence of infection for 7 or more days after ARDS onset. Persistent ARDS, also known as unresponding ARDS, is characterized by ongoing inflammation and disorganized fibrosis. 1– 5 The clinical consequence of these diverse injury responses is impaired oxygenation and ventilation. 1– 5 Abnormal physiological responses in ARDS include an increase in right-to-left intrapulmonary shunt, an increase in total dead-space fraction, and a reduction of lung compliance. Acute respiratory distress syndrome (ARDS) is a complex inflammatory lung injury associated with pulmonary microcirculation abnormalities and capillary thrombosis. ![]()
